Memory changes can appear slowly which may lead individuals to live with challenges for a long time without seeking a formal diagnosis. Delaying care can lead to unnecessary safety risks, high levels of family stress and missed opportunities for treatments that protect independence.

Craigwiel Gardens is proud to host a MINT Memory Clinic at our Community Connections Centre. This specialized program provides comprehensive, compassionate and proactive care to help you or your loved one live well at home.

What is a MINT Memory Clinic?

MINT stands for Multispecialty Interprofessional Team (or Team-Based Care). The clinic brings together a team of trained health care professionals at Craigwiel Gardens who will work with you and your referring physician.

When you use our clinic, you are backed by a trusted, evidence-based medical model. There are currently over 140 MINT Memory Clinics operating across Canada. This rapidly growing network has trained more than 2,000 health care professionals, including our team, providing highly specialized memory care closer to home. Studies show that early management through a MINT clinic dramatically improves quality of life and helps individuals delay or avoid a move into long-term care.

Who We Serve

The MINT Memory Clinic is available to individuals who are on a long-term care waitlist, or eligible to be on a waitlist.

The clinic provides thorough assessments, support and treatment planning for individuals experiencing:

  • Dementia
  • Mild cognitive impairment (early changes in memory or thinking skills)
  • Delirium (sudden confusion or changes in mental state)
  • Mental health challenges associated with memory loss

What to Expect at Your Appointment

Our goal is to help patients with memory difficulties maintain independent living and the best quality of life for as long as possible.

On the day of your appointment, you will meet with our team of skilled health care professionals, which includes a physician, nurse, pharmacist and social worker. Please allow two and a half to three hours for the appointment.

We assess and provide recommendations on many aspects of memory function, including bloodwork and X-ray tests that may be required, available treatments and driving safety. We will provide your family doctor with the results to help provide the best care for you.

During the assessment, we will provide a lot of information and encourage you to bring a family member or friend.

How to Get a Referral

The MINT Memory Clinic provides specialized, diagnostic medical care and requires a formal referral from a family physician or medical specialist.
