MINT 1Connections offers enhanced supports for living well to optimize health, maintain independence, support families and reduce emergency medical care. The program serves individuals who are currently on, or eligible for a long-term care waitlist.

By providing easy access to professional medical and physical expertise, Connections supports safe and healthy aging at home, while offering essential resources for their caregivers.

Our Services

Your service plan will be unique to you and your caregiver. Depending on your needs, your plan may include any of the following:

Clinical Care

  • Nursing and personal support: Wound care, foot care, head to toe assessments and vital signs monitoring.
  • Access to clinical support: Physiotherapy and occupational therapy, dietitian services (nutrition advice) and social work.
  • Health promotion education: Falls prevention strategies and everyday self-care guidance.

MINT Memory Clinic

MINT Memory Clinic is a team-based (interprofessional) program providing comprehensive memory care for individuals living with dementia and other memory impairments.

Early diagnosis and treatment can help maintain independent living, improve quality of life, reduce unnecessary hospital visits and delay long-term care placement.

Individuals who are on a long-term care waitlist, or eligible to be on a waitlist, must be referred to the MINT Memory Clinic through a physician or specialist.

  • What to expect: Please allow two and a half to three hours for the initial appointment.
  • What to bring: Please bring along a family member or someone who knows you well to your appointment to help our team get a complete picture of your health.
  • Preparation: We will send an information package to you and your family before your first visit.

Personal Support

  • Essential Care: Access to assisted bathing and toileting.
  • Self-Care Services: On-site hair salon services and staff assistance with laundry services.
  • Nutrition: Balanced, community-style meals served on-site (congregate dining).

Social and Recreational Programming

  • Active Living: Craigwiel Gardens is home to a Seniors Active Living Centre available to older adults for a nominal fee. It includes a fitness room and social and educational programs. To learn more, visit our webpage.
  • Cognitive & Spiritual Wellness: Arts, music, pet therapy and worship services to support emotional expression and spiritual connection.

Community Paramedicine

Craigwiel Gardens has partnered with Middlesex-London Paramedic Service’s Community Paramedicine program.

Community Paramedicine provides health care visits by specially trained paramedics who work alongside your primary care provider. They can assist with health assessments, chronic disease management, wound care, diagnostic testing such as bloodwork and ultrasounds, remote health monitoring, care coordination and referrals to community services. Their goal is to help clients stay healthy at home and reduce unnecessary emergency department visits and hospitalizations.

Caregiver Supports

  • Education & Resources: Peer support groups and expert-led sessions on safety and self-care topics.
  • Seamless Navigation: Our Service Coordination team simplifies the health care system, supporting caregivers with a single, trusted point of contact for questions.

Frequently Asked Questions

The program is open to individuals who meet any of the following criteria:

  • You are currently on the waitlist for long-term care (LTC).
  • You have been assessed as eligible for LTC by Ontario Health atHome.
  • You are soon to be eligible for LTC. This includes individuals currently awaiting an assessment by Ontario Health atHome, as well as individuals with specific health conditions who need these specialized services to help avoid a stay in the hospital or a premature move into long-term care.

No, you do not need a formal referral from a family doctor to join the Connections program. Individuals, family members or caregivers can reach out directly to book an initial consultation.

All clinics, specialized assessments and treatments are held in our dedicated, professional clinical spaces at Craigwiel Gardens’ Community Connections Centre, which also hosts our Adult Day Program and Seniors Active Living Centre.

Connections programs are offered at no cost. Some services may have a nominal fee.
