Craigwiel Gardens is committed to ongoing quality improvement in our Long Term Care Home, Craigholme.
Our Quality Improvement Plan (QIP) is a formal commitment to quality improvement, aligned with system and provincial priorities. By identifying and explaining how our Home is going to achieve our long-term improvement goals, we are able to effectively focus our quality improvement efforts on those key issues that will truly improve the care we provide for our residents.

The offical website of the municipality of North Middlesex. Formed in 2001 of five communities including Ailsa Craig, North Middlesex is the official township of Craigwiel Gardens. 

HCCSS helps assess patient care needs, and deliver in-home and community-based services to support your health and well-being. In addition, HCCSS provides access and referrals to other community services, and manage Ontario’s long-term care home placement process.

Our Nursing Home is licensed and inspected by Ontario’s Ministry of Health and Long Term Care. 

First announced by the Ministry of Health and Long-Term Care in 2011, BSO’s mandate is to reinvent the system of care for seniors across Ontario, their families and caregivers who live and cope with responsive behaviours associated with dementia, mental illness, addictions and other neurological conditions. We are pleased to be an active participant in this wonderful program.

A Power of Attorney is a legal document in which you give someone you trust (called your “attorney”) the right to make decisions for you if something happens and you are no longer able to look after matters on your own.

You can authorize your representative online using the “Authorize my representative” service in My Account and your representative will have instant access to your information and the online services to easily manage your account. If you choose to send us this form by mail, it will be processed within the service standards.

Registered Dietitians promote optimal, resident-focused, nutrition care by providing admission/quarterly/change of status assessments and nutrition care plan development. Nutrition counselling and consultation with family and other team members is also provided.

We strive to provide industry leading service by working along side your team to facilitate seamless care for your residents.
